Where do you get the fillable map that you can add to your signature line.

I see that lots of people have the fillable maps that they use on their signature line? I used to have one but cannot find it anywhere.

The link from the outgoing platform doesn't work anymore. Is there an updated process to add a rider "States Visited" map on the new platform?
