I have worked with some bolts and nuts with 4 to 6 inch diameter shafts, that held the bearings for ball mills or huge dryers. The bearing and bolts were lifted to the mounting platforms and set with cherry pickers. We tightened the bolts down with huge hammer wrenches and a 16 pound sledge hammer. Never worked with anything the size of the ones in the picture. Bad enough to swing a 16 pound sledge for a couple hours.

Bolts about that size, maybe larger, that are used to fasten down the lid on a nuclear reactor are not torqued. They are stretched, the nuts run down snug, and the stretcher is released. Good way to get the tension in them that is needed. Beats torquing for accuracy!
