That is the Old style Custom Dynamics's High Mount Tail/Turn light and YES I have one and feel it's very much worth it. Having followed my wife on our RT with my F-150 I realized how little the stock brake light showed when a truck pulls up close. The light is very bright and the turns are too. The new style doesn't have the chrome., it's all one LED light bar.
A top case light is an internal light that lights up the inside of the Top Case when opened at night.

Well that is a matter of personal opinion. The High Mount brake light was one of the FIRST farkels I installed on my and it has worked flawlessly for years. I have one from a different manufacturer and it looks OEM. It's VERY bright when the brakes are applied and quite visible with the ignition on.
